willing to work every other weekend**POSITION SUMMARY: - - - -CVS
Health is seeking a Nurse Practitioner (NP) to join our Primary
Care team. In this capacity, you will work in collaboration with a
dedicated clinical care team to provide evidenced-based care to a
panel of patients. The clinical care team will work with the
provider daily to drive better outcomes for entire panel of
patients. - This will include accurate assessment, diagnosis,
treatment, management of health problems, health counseling, and
disposition planning. This role will report to the practice
manager. - -PRIMARY DUTIES AND RESPONSIBILITIES: -
-Responsibilities include but are not limited to -Patient-Centered
Quality and Safety -
- Uses knowledge of etiologies, risk factors, pathophysiology,
presentation, and epidemiology of medical conditions relevant to
primary, acute, chronic, and transitional to provide care to
patients -
- Performs history and physical assessments of patients that are
accurate, complete, concise, relevant, and appropriate for age,
gender, and clinical symptoms. -
- Provides patient counseling; inclusive of pregnancy prevention,
STI Prevention/safer sex practices, contraceptive care counseling
and medication management - -
- Counsels and educates patients and families on health
maintenance and respond to patient care inquiries -
- Evaluates and treats patients' medical needs with safe and
high-quality treatment -
- Documents all patient care within an electronic health record
in accordance with current policies and procedures -
- Works as a member of the clinical care team providing care and
coordination of our patients with internal and external colleagues
ensuring the highest level of care is always provided for all
patients -
- Provides patient-centered care that is compassionate,
appropriate, and effective for the promotion of health, prevention
of illness and treatment of disease - -
- Refers to appropriate specialists and patient care components
as appropriate -
- Maintain patient confidentiality in accordance with PHI and
HIPAA standards - -Communication and Interdisciplinary

- Collaborates with clinical care team to ensure the delivery of
high quality, patient centric, longitudinal care -
- Participates in and in some cases leads clinical care team
huddles - -

- On call rotation shared with providers in market for after
hours patient clinical needs or shift coverage -

- Assist with hiring, development, and evaluation of Primary Care
clinical care team as appropriate -
- Complete necessary tasks for clinic operations, including but
not limited to following up on lab results, and receiving incoming

- Continuing education, including what is required to maintain
employment at CVS Health/ Primary Care, is the responsibility of
the provider - -EDUCATION: - - -Completion of master's degree level
Family Nurse Practitioner with current National Board Certification
and State of Employment license to practice in the Advanced
Practice role required. - -Required Experience: -
- A minimum of 1 year experience as a Nurse Practitioner is
required. ---Experience in Family medicine or primary care practice
preferred.--- -
- Internal applicants with less than 1 year experience are
eligible with successful completion of the CVS Minute Clinic
Primary Care Training Program -
- Active Advance Practice Registered Nurse, Nurse Practitioner
license in good standing -
- Current National Board Certification in Family Practice - -

the management of chronic disease conditions. -

RangeThe typical pay range for this role is:$41.85 - $90.13This pay
range represents the base hourly rate or base annual full-time
salary for all positions in the job grade within which this
position falls. -The actual base salary offer will depend on a
variety of factors including experience, education, geography and
