This full-time, permanent position is ideal for candidates with a solid job history, significant experience in home health, and a current RN license in Georgia. As an RN Manager, you will play a crucial role in delivering exceptional patient care and collaborating with a multidisciplinary team to ensure compliance with our agency's high standards.Key Responsibilities

• Accurately complete and maintain initial medication profiles for assigned patients, ensuring ongoing updates in medical records.
• Deliver skilled nursing care as outlined in the care plan, adhering to agency policies, including infection control and quality improvement standards.
• Collaborate with physicians to create and adjust care plans based on patient assessments and progress.
• Communicate patient updates and new orders effectively to clinicians, documenting interactions appropriately.
• Ensure timely and accurate documentation of payer changes and provide health education to patients and families.
• Manage care coordination for assigned cases, actively participating in case conferences and discharge planning.
• Conduct ongoing assessments of patient conditions and adapt care plans as necessary, while documenting incidents in the risk management system.
• Maintain a well-organized patient schedule in alignment with orders and authorizations.Required Skills
  • Current RN License in Georgia
  • CPR Certification
