Job DescriptionWe are seeking a dedicated Registered Nurse Case Manager to join our healthcare team. The ideal candidate will be responsible for coordinating patient care, providing medical assessments, and ensuring optimal healthcare outcomes.Responsibilities

• Manage and coordinate patient care throughout the healthcare continuum
• Conduct patient assessments and develop individualized care plans
• Collaborate with healthcare providers to ensure quality patient care
• Provide education to patients and families on treatment plans and disease management
• Oversee discharge planning and ensure smooth transitions between care settingsExperience
  • Current Registered Nurse (RN) license in the state of practice
  • Minimum of 2 years of experience in case management or a related field
