If you're looking to grow your career but also maintain a work-life balance that suits you, Agape Hospice is the place to do just that.ABOUT THE ROLEThe Registered Nurse Case Manger (RNCM) will provide direct patient care services to hospice patients and supportive care to the patient's family; manage an interdisciplinary group (IDG) consisting of Licensed Practical/Vocational Nurses, Certified Nursing Assistants and support staff to enhance the quality of hospice care; and case manages all assigned patient clinical records.*This schedule is M-F 8:30am-5pm, no weekends*ROLE REQUIREMENTS

• Graduate of an accredited school of nursing.
• A minimum of one (1) year experience in oncology, long term care, or hospice or home health.
• Current licensure in the state of residence.
• CPR certification.
